On August 8th, the topic of high heels came up in a hearing—stilettos, high heels, mid-heels, or even shallow ones. Women have always had a special connection with these shoes. But a new study from Iowa State University has uncovered a surprising fact: wearing high heels could increase the risk of developing osteoarthritis.
The research team examined how walking in high heels affects the joints in the lower limbs. They discovered that high heels transmit shock waves from the heel all the way up to the body, creating significant pressure on the knees. Prolonged use of high heels may lead to joint wear and tear over time. Danielle Buckma, who led the study, stated, “Our findings show that wearing high heels increases the risk of osteoarthritis. The higher the heel, the greater the risk.â€
To conduct the study, Ms. Buckma tested three types of shoes with different heel heights—flat, 2-inch, and 3.5-inch heels—on 15 women. Using sensors, accelerometers, force-measuring platforms, and cameras, she captured detailed movement and force data. These insights could one day influence how millions of women choose their footwear.
While previous studies have looked into the effects of high heels on joints, this research revealed that heel height alters walking patterns, such as reducing stride length and slowing down pace. The higher the heel, the more pressure is placed on the knee. Buckma explained, “Walking in high heels for extended periods can lead to joint degeneration and eventually cause knee osteoarthritis.â€
The findings will be presented at the annual meeting of the American Society of Biomechanics at Brown University. Professor Phil Martin, who collaborated with Buckma, said, “I believe her conclusions are accurate. Wearing high heels regularly puts people at risk, and the higher the heel, the greater that risk.â€
Buckma also found that high heels, especially those two inches or higher, alter body posture by shifting the position of the ankles, knees, hips, and overall body alignment. This can cause strain on the lower back. She noted, “The changes in posture are quite visible. We observed differences in joint angles, including knee and hip positions. Her research was inspired by her twin sister Ashley, who worked as a sales manager in a Chicago department store. She noticed that many female colleagues, especially older women, suffered from knee and hip problems due to daily high-heel use. However, Buckma doesn’t advise completely avoiding high heels. Instead, she suggests, “Try to wear moderate heels when possible, and opt for lower shoes if you can.â€
